partner Referrals
When a first responder identifies a child that has been exposed to violence or trauma, ACERT will be notified. ACERT will initiate the family advocate who will reach out to work with the family and identify appropriate agencies for referral. A signed release of information will be obtained from the parent/guardian and that release, along with the police report that was generated from the incident, will be shared with the social services agency or medical center. From there, the agency representative will reach out to the family for typical intake scheduling.
When addressing how agencies can connect formally with ACERT, consider the following:
ACERT Partnership Questions:
How would the partnership help to mitigate ACEs effects?
What population do they serve?
What services do they provide? Will there be resource capacity issues?
Are they open to engaging in addressing barriers to service (i.e. language, funding, transportation)?
Are they well planted and networked in the community?
Will they be available to make stakeholder meetings?
Are they agreeable to data collection?
Do they have legal considerations that may be different from other agencies?
